General Question
If you're going to ask just one question, ask an open ended question.

Specific Questions
If you're going to ask more than one question, be specific on the first two and general on the third.

Usability

• Was my website easy to navigate?
• Could you easily find what you were looking for on my website?
• Did you notice anything missing from my website that you expected to find?
• How long did it take you to determine what my website was about?
• Were any of the buttons confusing?
• Was it easy to find my contact information?
• What would you suggest I do to make my website easier to use?

Design

• Describe the overall look and feel of my website?
• Did my website fit on your computer screen?
• Did my website load properly on your computer?
• Did my video and/or audio play as expected?

Content

• What kind of information would you like to see available on my website?
• What part of my website did you enjoy the most?
• What's the most important piece of information I should have on my website?
• What would you suggest I do to make my website more valuable?
